Cholecalciferol

膽鈣化醇;維生素D3

Mechanism of action:

Cholecalciferol 是一種脂溶性維生素前驅物,主要功能是維持體內鈣、磷平衡並促進骨骼健康。皮膚本身就能合成,在紫外光 B(UV-B,290-315 nm)照射下,皮膚中的 7-脫氫膽固醇(7-dehydrocholesterol)能透過光化學反應與熱異構化轉變為 cholecalciferol。Cholecalciferol 在肝臟中能由細胞色素 P450 2R1(CYP2R1,vitamin D 25-hydroxylase)轉化成 25-羥膽鈣化醇(25-hydroxycholecalciferol,calcidiol),是血液中主要存在的維生素 D 型態。之後 25-羥膽鈣化醇會在腎臟由細胞色素 P450 27B1(CYP27B1,25-Hydroxyvitamin D3 1-alpha-hydroxylase)轉化成 1,25-二羥膽鈣化醇(1,25-dihydroxycholecalciferol,calcitriol),能與體內多種器官的維生素 D 受體(vitamin D receptor)結合,調控許多基因的轉錄,增加腸道的 Ca²⁺、PO₄³⁻ 吸收、讓腎臟維持血鈣,並透過促進骨細胞表現 RANKL 等方式,間接促進破骨細胞分化與活化以動員鈣離子(在需要時釋放 Ca²⁺)。
