Isosorbide dinitrate

二硝酸異山梨醇酯

Mechanism of action:

Isosorbide dinitrate 是一種有機硝酸鹽類血管擴張劑（organic nitrate vasodilator）。Isosorbide dinitrate 進入體內後會代謝成 isosorbide mononitrate，在血管平滑肌中經酵素轉化釋放一氧化氮（NO）。一氧化氮會刺激可溶性鳥苷酸環化酶（guanylate cyclase），使 cGMP 濃度上升。cGMP 促進肌球蛋白輕鏈去磷酸化，降低鈣離子對收縮蛋白的作用，導致血管平滑肌放鬆。隨後靜脈擴張，降低回心血量，降低心室壁張力與心肌耗氧量。
